Each subscribing senior adult receives a set of digital devices for self-monitoring. Depending on the pre-exixsting chronic conditions, we provide a blood pressure (BP) monitor, and a multi-parameter device (measures glucose, total cholesterol, uric acid etc).
The self-measurement of biometrics (blood pressure, glucose, cholesterol etc.) at home allows the measured data to be digitally recorded onto a health plaform for remote monitoring.


Senior adults' home self-measured biodata (blood pressure (BP), blood glucose (BG), total cholesterol (TCh) etc.) is digitally recorded onto a digital health platform on the cloud. A team of care professionals monitor the senior adult's recorded data, to identify any deviations from the normal.
Should any negative biodata changes be observed, an alert is sent to the treating clinician and advisory-contact is also made to the monitored senior adult as well. The advisory contact is meant to confirm safety of the senior adult and to ensure adherence to treatment. The digital health platform also performs biodata analysis that will help identify the level of health management.

Each senior adult has on registration share their treating health professionals details, who will be formally informed of their patient's use of the innovative health technology and the available health data.
The senior patient's self-measured biometrics (blood pressure, glucose, cholesterol etc.) monitored by our professional care team, is shared with the treating doctor either bi-monthly (as a health report) or in case of urgent abnormal deviations.
